sudah dicoba dengan theme lain? coba dengan theme standar woocommerce misalnya storefront. Atau coba non-aktifkan beberapa plugin sekiranya ada konflik dengan plugin lain.

Sepertinya konflik dengan plugin WooCommerce Checkout & Account Field Editor. Plugin tersebut saya non-aktifkan dan halaman checkout berfungsi normal (sukses melakukan order).
